As a member company, you can offer your employees the EyeMed Access Plan D discount only plan that provides reduced fees for exams, frames, lenses and contact lenses. EyeMed Access Plan D offers:

This plan is available for only $12.00 per employee including coverage for all elibible dependents up to two years, based on the employers' eyewear program anniversary date. The employer has the option of offering EyeMed on a voluntary employee pay or any combination of employer/employee premium contribution. The minimum employee participation is two full-time employees.
