Blue Access PPO
Most network freedom. This is Anthem’s broadest network of providers. Can be offered as an Anthem ByDesign Buy Up plan. HSA options are also available.

Blue Preferred^® Plus POS
Potential savings for employers, flexible provider choice for employees. Employees receive the highest level of benefits from network providers, but have the option to visit any provider for a greater share of the cost. Can be offered as an Anthem ByDesign Buy Up plan. HSA options are also available.

Blue Preferred^® HMO (for 51+ groups)
Most cost control for employers. Employees have a broad selection and direct access to network providers.

Business Options PPO/POS (for 2-50 groups)
